Wrong tense and wrong phrasing.

correction:
1. The teacher asked if i had any problem (Though it may sound strange, but trust me, it is correct.)

B.The teacher asked me what the weather was like that day. (wrong)

Wrong Phrasing

Correction: The teacher asked me what was the weather like on that day.

C.The teacher asked me what was wrong with me. (Wrong)

Awkward expression.

Correction: "what is wrong with you?"the teacher asked.

D. The teacher asked me what the matter was with me. (Acceptable)

This is better as compared to the rest, however, people seldom speak this way for it is not very coherent nor fluent.

Conclusion: All the four choices have more or less some flaws, be it in grammar or phrasing. But i think if you use more english in your daily conversation or watch english drama, you will be able to distinguish clearly between right and wrong.
